What Is an Odds Ratio?

An Odds Ratio (OR) is a measure of association between an exposure and an outcome. It represents the odds that an outcome will occur given a specific exposure, compared to the odds of the outcome occurring in the absence of that exposure.

An $OR = 1.0$ indicates no association. An $OR > 1.0$ suggests the exposure is associated with higher odds of the event (risk factor), while an $OR < 1.0$ implies lower odds (protective factor).

When to Use It

Case-Control Studies: The primary effect size metric when prospective incidence rates cannot be measured directly.
Logistic Regression: Exponentiating logistic regression slope coefficients ($\beta_j$) to obtain adjusted odds ratios ($e^{\beta_j}$).
Clinical Trials & Meta-Analyses: Comparing binary success/failure rates across experimental arms.
Epidemiological Risk Assessment: Quantifying disease likelihood associated with environmental or genetic risk factors.

The Mathematical Formulas

Where $a, b, c, d$ represent cell frequencies from a $2 \times 2$ contingency table. If any cell is zero, an automatic Haldane-Anscombe correction ($+0.5$) is applied to allow finite estimation.

Worked Example

Scenario: 100 subjects: Exposed ($a=40$ event, $b=10$ no event); Unexposed ($c=20$ event, $d=30$ no event) at $\alpha=0.05$.

Step 1: Compute Odds Ratio ($OR$):
$OR = \frac{40 \times 30}{10 \times 20} = \frac{1200}{200} = 6.0000$.

Step 2: Log Odds Ratio & Standard Error ($SE$):
$\ln(OR) = \ln(6.0) = 1.7918$
$SE = \sqrt{\frac{1}{40} + \frac{1}{10} + \frac{1}{20} + \frac{1}{30}} = \sqrt{0.025 + 0.10 + 0.05 + 0.0333} = 0.4302$.

Step 3: 95% Confidence Interval:
$\ln(\text{CI}) = 1.7918 \pm 1.96(0.4302) = [0.9486, 2.6350]$
$95\% \text{ CI for } OR = [e^{0.9486}, e^{2.6350}] = [2.5822, 13.9434]$.

Conclusion: Since the 95% CI does not include 1.0 ($p < 0.0001$), exposure is significantly associated with higher odds of the event.
